Should a women's rash vest fit tightly or loosely?

A performance rash vest should fit close enough that it does not balloon in the water or bunch beneath a wetsuit, but it should not restrict breathing or shoulder movement. Does a UPF 50+ rash vest replace sunscreen?

No. UPF fabric protects the skin it covers, but exposed hands, face, neck and legs still need suitable broad-spectrum sunscreen and regular reapplication. Protection also depends on coverage and condition: a long-sleeve, high-neck vest covers more than a short-sleeve style, while stretched, worn or very wet fabric may not perform exactly like a new garment.
